a. The WUSN “Chicago Blackhawks Ticket Giveaway” Website Contest (the “Contest “) entry period will begin on Monday, January 25, 2010 at approximately 12:00 AM Central Time (“CT”) and end on Friday, April 2, 2010 at 12:00 PM CT. The Contest will end when the winner is selected at approximately 1:00 PM CT on Friday, April 2, 2010.
b. To participate in the Contest, you must enter via the following method: Beginning on Monday, January 25, 2010 at 12:00 AM CT, log onto www.us99.com, click on the “Contests” page link, and click on the “Chicago Blackhawks Ticket Giveaway” contest link. Enter your first and last name, complete address, city, state, zip code, home telephone number, date of birth, and a valid email address in the online entry form. All entries must be received by 12:00 PM. CT on Friday, April 2, 2010 to be eligible for drawing on that day. Contestants will have from 12:00 a.m. CT on Monday, January 25, 2010 until 12:00 p.m. CT on Friday, April 2, 2010 to qualify for the drawing on Friday, April 2, 2010.
c. One (1) Internet entry per person and one (1) Internet entry per email address are permitted. Internet entries will be deemed made by the authorized account holder of the email address submitted at the time of entry. The authorized account holder is the natural person who is assigned to the email address by an Internet access provider, online server provider or other organization that is responsible by assigning email address or the domain associated with the submitted email address. For purposes of these Official Rules, “receipt” of any online entry will be solely determined when the Contest administrator’s website server records the entry date. Any automated computer form reply or receipt (such as one confirming delivery of email) does not constitute actual receipt by Administrator of an entry form for purposes of these Official Rules. The WUSN computers are the official time keeping devise for the Contest.
d. WUSN is not responsible for entries not received due to difficulty accessing the Internet, service outage or delays, computer difficulties or other technological glitches.
a. The Contest is open to all persons who are at least eighteen (18) years old at the time of entry and reside in the Chicago land area. Employees of WUSN-FM, the Chicago Blackhawks, its licensee, or any of their respective affiliates, subsidiaries, advertising agencies, or any other company or individual involved with the design, production, administration, execution or distribution of the Contest (collectively known as the “Contest Entities”) and their immediate family (spouse, parents and step-parents, siblings and step-siblings, and children and step-children) and household members (people who share the same residence at least three (3) months out of the year) of each such employee are not eligible. The Contest is subject to all applicable federal, state and local laws and regulations.
b. Participation constitutes entrant’s full and unconditional agreement to these Official Rules and Sponsor’s decisions, which are final and binding in all matters related to the Contest. Winning a prize is contingent upon fulfilling all requirements set forth herein.
c. Winner must hold a valid government-issued photo-ID and sign and return any required Affidavit of Eligibility and/or Publicity/Liability Release in order to claim prize.
d. Contestants are required to provide truthful information and WUSN will reject and delete any entry that it discovers to be false or fraudulent. WUSN will disqualify any entry from individuals who do not meet the eligibility requirements, and WUSN will also delete any entry knowingly received from persons under the age of thirteen (13) in compliance with the Children’s Online Privacy Protection Act.
a. One (1) prize will be awarded. The prize is two (2) tickets to the Chicago Blackhawks Game at the United Center, located at 1901 W. Madison Avenue, Chicago, Illinois, on Sunday, April 11, 2010. Approximate Retail Value (“ARV”) of the prize: $240.00 ($120.00 per ticket). Winner and guest are subject to the terms and conditions as set forth by issuer of tickets. The Contest Entities in their sole discretion, reserve the right to deny entry or remove winner and/or guest from premises if they engage in disorderly conduct or harass anyone while at the event.
b. There is no substitution, transfer, or cash equivalent for prizes, except that Contest Entities may, at their sole discretion, substitute prizes of comparable value or cash. The prizes are expressly limited to the item(s) listed above. Other restrictions may apply.
c. Prize will be mailed to the winner at the address listed on their entry form, and assumes the risk of its loss. The winner will forfeit any prize or prize certificate not claimed within five (5) days of winning, and an alternate winner will not be selected. WUSN is not responsible for the safe arrival of a prize or prize certificate.
a. Decisions of WUSN management with respect to the Contest are final.
b. One (1) prize winner will be selected at approximately 1:00 PM CT on Friday, January 2, 2010 and will receive two (2) tickets to the Chicago Blackhawks Game at the United Center, located at 1901 W. Madison Avenue, Chicago, Illinois, on Sunday, April 11, 2010. Approximate Retail Value (“ARV”) of the prize: $240.00 ($120.00 per ticket).
c. Odds of winning depend on the number of eligible entries received by the end of the entry deadline.
d. Winner need not listen nor be present to win.
e. Winner will be contacted by the WUSN Promotions Department by phone on Friday, April 2, 2010 following the drawing. If winner cannot be reached within three (3) business days of the initial notification attempt and does not execute and return any required Affidavit of Eligibility and/or Release of Liability Prize Acceptance Form by 4:00 PM CT on Friday, April 9, 2010, the prize will be forfeited and an alternate winner will not be selected. Potential Winner must continue to comply with all terms and conditions of these Official Rules. Winning is contingent upon fulfilling all requirements. If the potential winner of any prize cannot be contacted, fails to sign and fax the affidavit of eligibility and liability/publicity release within the required time period (if applicable), or if the prize or prize notification is returned as undeliverable, potential winner forfeits the prize.
a. Payments of all federal, state and local taxes are solely the responsibility of the winner. The winner will be required to complete and submit an IRS Form W-9 with the winner’s full Social Security Number or the equivalent for receipt of any prize valued at $600 or more or for any prizes awarded by the Station (or any other CBS Radio station in Station’s market) in a calendar year with an aggregate value of $600 or more. Failure to submit a complete W-9 or equivalent will result in forfeiture of the prize. Such winnings of $600 or more will be reported to the IRS.
b. By participating in the Promotion, the winner agrees to have the winner’s name, voice and likeness used in any advertising or broadcasting material relating to the Promotion without additional financial or other compensation, and, where legal, to sign a publicity release confirming such consent prior to acceptance of the prize.
c. Prior to awarding any prize or prize certificate, the Station, in its sole discretion, may require Promotion winner (and any travel companion(s) or guest(s)) to sign a liability release, agreeing to release and hold harmless the Station, its licensee, the licensee’s parent, subsidiary and affiliated entities, their respective officers, shareholders, directors, employees, agents and representatives and all of their successors and assigns from and against any and all claims or liability arising directly or indirectly from the prize and participation in the Promotion.
d. If for any reason this Promotion cannot be executed as planned, including, but not limited to, as a result of infection by computer virus, tampering, unauthorized intervention, fraud, technical failures, or any other causes beyond the control of the Station that corrupt or affect the security, administration, fairness, integrity or proper conduct of the Promotion, or if the Promotion is compromised or becomes technically corrupted in any way, electronically or otherwise, the Station reserves the right to cancel, terminate or modify the Promotion. If the Promotion is terminated before the original end date, the Station reserves the right to select the winners by random drawing from among all eligible non-suspect entries received as of the termination time/date or otherwise modify the procedure for selecting winners.
e. The Station, in its sole discretion, reserves the right to disqualify any person tampering with the entry process or the operation of the Station’s website. Failure to comply with the rules of the Promotion may result in a contestant’s disqualification and/or forfeiture of any prize or prizes. If the Station makes a good faith determination that an entrant has cheated or committed fraudulent activity in connection with a Promotion, the Station reserves the right to disqualify that entrant from entering and/or winning future Promotions and to prosecute and seek damages to the fullest extent permitted by law.
f. The Station reserves the right to make changes in the rules of the Promotion, including, without limitation, the substitution of a prize of equivalent value, which will become effective upon announcement. If due to circumstances beyond the control of the Station, any competition or prize-related event or travel is delayed, rescheduled, postponed or cancelled, the Station reserves the right, but not the obligation, to cancel or modify the Promotion and shall not be required to award a substitute prize.
g. The Station is not responsible for typographical or other errors in the printing, the offering or the administration of the Promotion or in the announcement of a prize.
i. Contest Entities reserve the right to cancel, suspend and/or modify the Promotion, or any part of it, if any fraud, technical failures or any other factor beyond Contest Entities’ reasonable control impairs the integrity or proper functioning of the Contest as determined by Contest Entities in their sole discretion. Contest Entities reserve the right in its sole discretion to disqualify any individual it finds to be tampering with the entry process or the operation of the Contest or to be acting in violation of these Official Rules or any other promotion or in an unsportsmanlike or disruptive manner. Any attempt by any person to deliberately undermine the legitimate operation of the Contest may be a violation of criminal and civil law, and, should such an attempt be made, Contest Entities reserve the right to seek damages from any such person to the fullest extent permitted by law. Contest Entities failure to enforce any term of these Official Rules shall not constitute a waiver of that provision.
